We know your time is valuable, so to help expedite your consultation, we kindly ask our patients to bring all of the following information with them to their first appointment:
- Your Medical Insurance Card and picture ID
- A copy of your most recent sleep study
- A referral or prescription from your primary care or sleep physician (if available)
- A copy of your dental x-rays (if available)
To further speed up your treatment on the day of your visit, you can also:
- Call us with your medical insurance information that’s on your card.
- Fax us a copy of your most recent sleep study.
- Fill out the Office Registration and Medical History forms before your visit.
Consult
We strive to make the consultation and examination process as comfortable, convenient, and stress-free as possible. We will start by reviewing your medical history, followed by an examination of your teeth, facial muscles, jaw, airway, and surrounding tissues.
After the examination, we will discuss treatment options for Obstructive Sleep Apnea (OSA) with you. We value open communication, so please feel free to ask any questions, and we will address them thoroughly.
To support your overall health, we will send all final documentation to your referring physician, primary care physician, and insurance provider.
